Description of original award (Fiscal Year 2022, $340,000)
The Hudson Partnership Care Management Organization (CMO) will use funding to enhance its current mentorship program for youth involved in the justice system and those at-risk for becoming involved. In five Jersey City schools, specialized Mentors will work with school personnel to identify youth who present with behaviors or challenges that may increase the likelihood of them entering the justice system or becoming more deeply involved in the system, and with youth who are currently on probation or who are presently detained but are expected to return to the community. When appropriate, Mentors will assist caregivers with referrals to Care Management services for youth who are in need of clinical services and supports. Mentors will also link youth to a calendar of engagement events and activities for the entire school year.
