Hillsides' 'Youth Moving On' is a program that provides housing and an independent living program for youth with special needs. The primary target population is youth who are leaving from group or foster family homes. The program will provide affordable housing and an independent living program to former foster youth with emotional or mental disorders. The project seeks to serve 20 youth, 18-21 years of age over a three-year period. Staff provide residents with physical and mental health therapy, training in independent living skills, and setting and attaining educational and employment goals. Progress will be measured by tracking each client's success in achieving these goals. The project will track clients for 24 months after completion of the program and monitor employment rates, education, and living arrangements.
