Transitions Project

Youth Services System (YSS), Inc. will empower a team to offer aftercare services to youth and families discharged from juvenile detention, emergency shelter, and those completing outpatient substance abuse treatment. In addition to aftercare and family support for these youth, Youth Services System will work with youth diagnosed as mentally retarded/ developmentally disabled [MR/DD] who are in state's custody to improve their opportunities to live independently after their discharge from foster care. This project will use existing materials including the Daniel Memorial assessment and the Phillip Roy curriculum to identify, strengthen, and teach skills for independent living. NCA/NCF
